Canadian Dental Care Plan (CDCP)

We are accepting patients on the CDCP.

At Evergreen Dental Care, we're excited to provide dental care for those on the federal dental plan.

How it works at Evergreen.

Fee Guide

We follow the current fee guide provided by the Ontario Dental Association (ODA) for most treatment.

The CDCP contribution towards your care is LESS than the ODA fees.  The difference between OUR fee and the Canada Dental Care Plan contribution will be billed at the day of treatment.

Additionally, patients are responsible for paying the 'Co-Payment' as determined by the eligibility for the CDCP based on family income.

Our staff is ready to help make this process easy and clear for you. We wish to be transparent and to help answer any questions you may have.

Children or Adults with Disabilities

Children under the age of 18 and adults with valid Disability Tax Credit certificates are now eligible to apply for the CDCP through Health Canada's website.

Those with benefits through Healthy Smiles Ontario (HSO) or the Ontario Disability Support Program (ODSP) can apply for the CDCP, and their benefits will be co-ordinated between the CDCP and the applicable provincial program.

If you currently have coverage under ODSP or HSO and are eligible for the CDCP, you must apply for the CDCP for treatment at our office.

Coverage

The Canada Dental Plan does NOT cover all recommended procedures. We can assist you with determining what coverage you have for any procedures. However, please be aware that your health needs are not dependent on your coverage.

The Canada Dental Plan does not cover the recommended frequency of cleanings, exams, and other procedures. Please be aware that for most adults, the recommended frequency for these will be higher than what is covered, so additional required treatments would be out of pocket.

Expenses

The out-of-pocket expense depends on the specific treatment. For some procedures, the CDCP contributes over 90% of the overall fee. For others, the contribution is less.

If you qualify for 100% coverage under the plan, your out of pocket expense is simply the difference between the contribution by the CDCP and the ODA fee guide suggested fee.

If you qualify for 40 or 60% coverage under the plan, your out of pocket expense will be the Co-Payment (remainder of the CDCP fee) plus the difference between the CDCP fee and the ODA fee guide suggested fee.

Your ESTIMATED out of pocket expense is payable at the time of treatment. It may not be possible to know the exact amount at the time of treatment until it is submitted to Sun Life but any overpayment will be reimbursed to you as soon as possible. Any underpayment will be billed to you to be paid at a later time.

Eligibility

The Canada Dental Care Plan coverage runs from July 1 to June 30 (12 months). You must re-apply every year to ensure eligibility. It is your responsibility to ensure you are enrolled and to provide that information to us before your appointment.
